Corticosteroid Injection for the Treatment of Morton's Neuroma: A Prospective, Double-Blinded, Randomized, Placebo-Controlled Trial.

Introduction

Morton’s neuroma는 흔한 forefoot pain의 원인이지만 orthotics, 신발 교정 등 보존적 치료에 치료 반응이 없는 경우가 흔하다. corticosteroid 주입술이 고려되나 치료의 반응에 일관성이 없다. 2004년도 systematic review에서 효과가 있다고 알려져 있으나 1년 시점에서는 대조군과 비교시 우월성이 입증되지 못했다. Morton’s neuroma에 대한 대부분의 연구는 case-control 또는 retrospective study가 대부분이다. 따라서 본 연구에서 double-blinded randomized, placebo-controlled 연구를 진행 하였다. corticosteroid + local anesthesia와 local anesthesia only 두 군으로 주사 치료 후 통증 경감을 주 결과로 확인하고자 하였다. 나이, 성별, BMI, 발생위치, 유병기간, 크기 등이 치료에 어떤 영향을 주는지 추가로 알아보았다.

Method

2013년 1월부터 2015년 1월까지 2년간 Morton’s neuroma를 진단받은 환자를 대상으로 하였으며 진단 기준은 1) paresthesia in the second and/or third interdigital space of the foot, 2) worsening of symptoms upon weightbearing or with certain/tight footwear, 3) clinical improvement after removal of conflicting footwear or after massage, 4) positive metatarsal compression test (painful), or 5) a palpable and/or painful click (positive Mulder’s sign)으로 하였다. 총 74명 환자에서 41명이 참여하여 실험군 20명 ((1 mL mepivacaine 2% +1 mL triamcinolone 40 mg), 대조군 21명 (2 mL of mepivacaine 2%)으로 V1 첫 외래 방문 및 1회 주사, V2 1주 뒤 2회 주사, V3 1회 주사 2주 후 3회 주사, V4 1회 주사 3개월 후, V5 1회 주사 6개월 후 진행 되었다.

Result

실험군에서 주사 후 1주 째 통증 경감이 유의하게 나타났으나, 3개월 및 6개월 시점에서 두 군 간의 통증 (visual analog scale for pain)의 차이, 기능적 향상 (American Orthopaedic Foot & Ankle Score (metatarsophalangeal/interphalangeal score))은 없었다. 연구 종료 시점에서 17명 (48.5%) (7명 실험군, 10명 대조군)이 수술적 치료를 받았다.

유병기간이 짧을수록, 나이가 많을수록 치료 경과가 좋았고, 치료 전에 radiating pain이 없는 경우 6개월 후 치료 성적이 더 좋았다.

Conclusion

Morton’s neuroma 치료로 corticosteroid + local anesthetics 주사 요법이 only local anesthetics 주사 요법과 비교 시 통증 경감 및 기능 회복에 우월하지 않다.
