Treatment outcome of local injection of botulinum toxin for claw toe: Differences between cerebral hemorrhage and infarction

목적: Claw toe에서 botulinum toxin 주사치료에 좋은 효과를 보이는 뇌졸중 환자군이 있는 반면, 그렇지 않은 군이 있다. 이것이 stroke type (hemorrhage vs infarction)의 차이인지 확인해본다. 

방법: Flexor hallucis longus (FHL) 과 Flexor digitorum longus (FDL) 근육에 보툴리눔 독소 주사치료한 뇌졸중환자들을 retrospective하게 분석해보았다. 58명 환자 (mean age, 61.4 ± 10.3 years) 가 분석되었고 평균 뇌졸중 유병기간은 6.7 ± 4.4년 이었다. stroke type별로 cerebral hemorrhage (n = 38, 66%), cerebral infarction (n = 20, 34%) 이였다. 평균 보툴리눔 독소 주사용량은 FHL에서 42.9± 22.3 units이였으며, FDL에서 35.3 ± 16.8 units이였다.

결과: Brunnstrom stage와 modified Ashworth scale 로 확인한 symptomatic improvement에서 infarction group이 hemorrhage group 대비 5.8배 높은 증상호전확률을 보였다. (OR = 5.787, 95% CI = 2.369–14.134, p = 0. 000)

결과에 영향을 미치는 것으로 알려진 요인들 (경직 및 마비 정도, 보툴리눔 독소량, 발가락 움직임의 FDL 근육 조절 정도)을 adjustment 후에도 cerebral infarction 환자군에서 대략 8.2–18.8 배 치료 효과가 더 높았다.

결론: Claw toe spasticity에서 보툴리눔 독소 주사 치료 반응은 hemorrhage patient보다 cerebral infarction patient에서 더 효과적이다.

Comment : 상기연구에서 뇌경색 환자에서 뇌출혈 환자에서 보다 claw toe 경직에 보툴리눔 독소 치료가 더 효과적인 명확한 이유를 제시하지는 못하고 있다. 다만, 출혈성 뇌졸중환자에서 일반적으로 경직이 더 심하기 때문일수 있음을 지적하고, modified Ashworth scale이 증가할수록 dose dependent Botulinum toxin 효과를 얻기 위해, higher dose를 사용함이 바람직하다고 주장하고 있어, 임상에서 이를 고려하여 claw toe 치료한다면 보다 만족스러운 효과를 얻을 수 있을 것으로 생각된다.
